Imagine a symphony orchestra where each musician plays a distinct role. In your body, insulin is the tireless conductor, coordinating the music of metabolism within each cell. But where does this maestro come from?
The insulin story begins on the short arm of chromosome 11, a tiny segment in the vast expanse of our genetic material. A preproinsulin molecule is synthesised in the rough ER of the pancreatic β cells. Much like a student progressing in their musical career, preproinsulin matures, transforming into an 86 amino acid peptide, proinsulin, due to the cleaving enzymes’ actions.
Proinsulin is then ushered into the Golgi complex, akin to a backstage dressing room. Here, proinsulin transforms into the star performer – insulin, a biologically active hormone, and a 31 amino acid C-peptide. A troupe of backstage crew facilitates this process, the enzymes: prohormone convertases 1 and 2 (PC1 and PC2), and carboxypeptidase-H.

In healthy β cells, over 95% of proinsulin perfects its performance, turning into insulin and C-peptide. Insulin is a peptide molecule composed of chain A (21 a.a) and chain B (30 a.a), linked together like two partners in a dance, with a molecular weight of just 6 kDa.
The finale? Insulin and C-peptide, in equal proportions, exit the cell and enter the bloodstream, bringing harmony to our body’s energy utilization.
Though small, insulin’s grandeur is reflected in its association with more Nobel Prizes than any other molecule. These include awards for its isolation (Banting and colleagues, 1923), sequencing (Sanger, 1958), atomic structure identification (Dorothy Hodgkin, 1964), and insulin-sensitive measurement technology (Yalow, 1977). A standing ovation indeed!
